To help you find your way through the process of beginning a counseling relationship I will try to reduce it to a few easy to follow steps.
STEP ONE:
Take a few moments to read the linked information forms on this page:
- Questions to Ask your Insurance Company
- Explanation of Services
- Client Intake Form
- Notice of Privacy Rights
- Consent for Treatment
- Form for Billing Insurance
After reviewing the information found on these information forms you may still have questions about me and the kind of counseling you can expect when you work with me. It will be helpful to have these questions answered to your satisfaction. Please contact me by phone or email and I will be happy to talk with you or discuss your questions with no obligation. Phone: 360-379-4849; email: pccpt@qwestoffice.net
After this discussion you may be ready to set an appointment. If you set an initial appointment with me, you are ready to move to step two.
STEP TWO:
If you have health insurance that will cover the costs all or part of the services provided, you may need to review what your coverages are and what requirements your health plan has. Talking with the Customer Service department of your insurance plan will give you the information you need. [refer to Questions to Ask your Insurance Company]. Pay particular attention to the need for a physician's referral, and number of sessions that will be covered.
Please print out the Notice of Privacy Practices and the Client Information Form. [One form for each person who will be attending counseling sessions]. These forms will need your information and signature. Fill them out to the best of your ability. Please bring these to your first appointment.
STEP THREE:
Please refer to the MAP link to help you locate the Presbyterian Counseling Center. The first appointment will be approximately 90 minutes in length. This time is spent orienting you to how we will work together, answering questions, and beginning to discuss 'what is bringing you to counseling.'
I welcome you to my practice and look forward to working with you...
